  Went in to get a form filled out. Explained over the phone and in person what and why and was told no problem. 6 days later receptionist (after multiple follow up calls on my part) says nope doctor won't talk to you and won't fill it out for you either and then just hung up.
  Called back thinking it was a disconnect, nope she hung up again.
  
  I'm not sure why it's a problem suddenly and why they didn't tell me no a week ago. Total waste of time and a copay.
